Individualized Supported Living

Live in your own home with support sized to your needs, from a few hours a week to 24 hours a day.

Individualized Supported Living (ISL) supports an adult in their own home, such as a house or apartment, instead of a group home or facility. The amount and type of supervision and training are set to fit the individual rather than a fixed program.

Support is based on assessed need and ranges from a few hours a week to 24 hours a day. The level can be adjusted over time as needs change. Insight assigns a consistent staff team, so the people providing support know the person and their routine.

Each plan is built around the individual: daily routines, personal goals, and the specific help required to live as independently as possible. Insight handles staffing, training, and day to day coordination so support is reliable and in place.

Who this service is for

Adults with developmental disabilities who want to live in their own home rather than a group home or facility. Support ranges from a few hours a week to 24 hours a day, for anyone eligible for Missouri Division of Developmental Disabilities services.

Eligibility

ISL is funded through Missouri's Medicaid Home and Community Based Services (HCBS) waivers, administered by the Department of Mental Health (DMH), Division of Developmental Disabilities. It requires a Division of DD eligibility determination, MO HealthNet (Medicaid) eligibility, and a support coordinator. If you already have a support coordinator, we can start there.
